This guide has been prepared by the Wisconsin Insurance Commissioner's Office and must be given to all prospective buyers of long-term care insurance at the time an application is taken or when long-term care insurance is solicited, whether or not the solicitation results in a sale.

If you have questions or concerns about your insurance company or agent, write to the insurance company or agent involved. Keep a copy of the letter you write. If you do not receive satisfactory answers:
