Registration for Vehicle Protection Product Warranty

The following are documents which must be completed and returned with the Initial Registration for Vehicle Protection Product Warranty. Any questions about the application should be directed to Mr. Steven Junior (608-267-4388) or Yvonne Sherry (608-266-0091).
Also included is information detailing the procedures for submitting the warranty contracts for approval. All warranty contracts and, if applicable, the warranty reimbursement insurance policy must be approved before the certificate of registration will be issued. Any questions about the warranty contract submission procedures should be directed to Ellen Vigil at (608) 267-1234.
No registration will be considered completed until all items on the registration form have been completed and all required forms have been completed, filed, and approved.
Warrantors shall make all records concerning transactions regulated under s. 100.203, Wis. Stat., available to the commissioner upon request. A warrantor's records shall include all of the following:
- Copies of all warranties under which the warrantor is obligated.
- The name and address of each warranty holder to whom the warrantor is obligated.
- The dates, amounts, and descriptions of all receipts, claims, and expenditures related to the warrantor's warranties.
- A warrantor shall retain all required records pertaining to each warranty holder to whom the warrantor is obligated for at least 2 years after the specified period of coverage has expired. A warrantor discontinuing business in this state shall maintain its records until it furnishes the commissioner satisfactory proof that it has discharged all obligations to warranty holders in this state.

Vehicle Protection Product Warranty Contract Submission Instructions
The warranty contracts should be submitted to the Office of the Commissioner of Insurance to the attention of Rhonda Peterson, Market Regulation. The subject line should indicate, in bold letters, "Vehicle Protection Product Warranty."
Warranty Reimbursement Insurance Policy Submission Instructions
The warranty reimbursement insurance policy must be filed with and approved by the Office of the Commissioner of Insurance, using the usual policy forms submission procedures. The insurers are reminded that the product category is "Liability" and the product code is "CTL." For ease in identifying the policy, the subject line should indicate, in bold letters, "Vehicle Protection Product Warranty Reimbursement Insurance Policy."
NAIC Company Code ApplicationFor Wisconsin-Domiciled Vehicle Protection Product Warranties Only
A link to the NAIC Company Code Application form is provided below for your convenience. Note, however, that you will need an Adobe Acrobat Reader to view or print the application form, which you can download at no cost from Adobe . Vehicle Protection Product Warranties need not file financial data with the NAIC, but are required to obtain a NAIC Company Code. This assists with the collection of demographic statistics by state and federal authorities. Once the NAIC Company Code is issued, it remains in force for the entire existence of the Vehicle Protection Product Warranty. Wisconsin cannot coordinate issuance of a NAIC Company Code to a Vehicle Protection Product Warranty that is incorporated in a state other than Wisconsin.
