Wisconsin insurance law requires that each intermediary (agent) shall, within 30 days, notify the commissioner in writing of any change in the intermediary's name, residence address, mailing address, or business address.
Each insurance intermediary firm licensed shall, within 30 days, notify the commissioner in writing of any change in its business or mailing address or change in the name and address of the designated representative.
An agent or firm can submit an address change or name change to firstname.lastname@example.org.
Telephone requests for name or address changes of are not accepted.
Once the name or address change request has been processed, agents or firms can print a copy of the updated license free of charge at oci.wi.gov.
